Documentation needed to get a passport
You will need 2 identical color passport photos, 2” by 2” in size, taken within the last 6 months, showing current appearance. Photos need to have a plain white background. The person must be looking straight ahead. No hair covering or adornments can be visible in the hair. Light sensitive glasses that obscure the eyes cannot be worn. To make this a one stop center, the Friends of the Costa Mesa Libraries offers Photo Service during Passport hours.
Cost: $10.00 for 2 identical photos. (cash, check or money order)
You will need the DS-11 form to submit your application. The forms are available at the library, or you can download them from the Passport Agency website: http://travel.state.gov. Be Prepared to supply: Social Security Numbers, current and/ or previous spouse’s birth information and parent’s birth information (even if deceased).
No photocopies accepted. You will need either a certified copy of a Birth Certificate issued by the county, city, or state of birth, or current or previous U.S. Passport or original Certificate of Naturalization. Documents must meet the requirements set by the Department of State. They will determine the acceptability of all documents presented.
Hospital Birth Certificates and California and Texas Birth Abstracts are NOT accepted for proof of citizenship. If you have either of the above you can order a new full certified copy by going in person to the Clerk-Recorder’s office in Santa Ana for Orange County or Norwalk for L. A, County or go through Vital check at: http://www.vitalchek.com.
Adults may show a current valid U.S. driver’s license or state issued Identification card, Military I.D. card (current) or Government issued photo I.D. card (current). For Out-of-State I.D., a second form of identification is required (Costco, AAA, bank or credit card).

You can apply for both documents (Passport Book and Passport Card) at the same time.
All passport applicants must apply in person, regardless of age.
Both parents must be present and show identification when applying for their child’s passport when the child is under age sixteen.
If one parent is unable to come, a Statement of consent form DS-3053 from the parent unable to come can be used, but must be notarized.
A birth certificate must also be present when renewing a child’s passport if the child is under the age of sixteen.
For an additional $60, you can expedite your passport to receive it within three weeks.
If you are traveling within two calendar weeks, you must call the Los Angeles Passport Agency at 1-877-487-2778 to make an appointment for service. You will need to show proof of travel.
